Page 1 APPLICATION FOR Membership Rainbow Grocery416 East HowardSouth Bend, Indiana46617I hereby apply for membership in the above unincorporated association andsubmit herewith a one year membership fee of S5.00 (non-refundable).I understand Rainbow Grocery will- Buy selected merchandise for me, other members and patrons and resellthem to us at a mark-up.- Allow ae to receive a further discounts on Rainbow Grocery goods inexchange for service rendered.- Serve members and the community.- Give me a chance to help decide basic policies, elect a board ofdirectors, and review operating budgets - through participationin membership meetings.- Give me a chance to help further Rainbow Grocery as a more cooperativeventure.- Provide members and patrons with sound and impartial consumer information free of all promotions.I agree to:- Invest at least five ($5.00) dollars more each succeeding calendarquarter until I have invested a total of fifteen ($15.00) dollars ina life-tine deposit.
